A glitch in an online survey replaced the word 'yes' with 'forks'


Dating back to at least early 2023, a bizarre and alarming technical glitch started popping up in some organizations’ online surveys and forms.

We hope this explanation will be helpful to you if you’ve encountered a similar issue, and we’re grateful to our survey respondents for alerting us to the error so it could be fixed.
